 I understand any form of donation from 4 Paws Pet Pantry or any entities associated with 4 Paws Pet Pantry will not be held liable for the recipient(s) or pet(s) health due to change in food, anything associated to food intake or any other harm/effect that could arise from various forms of donations. Food may be expired or open bag from distributor.  Donations will only be distributed to one person per household.  If it is found that more than one person from same household is registered, that will be considered fraud and each party will have their Pantry benefits terminated. Once signed up with the Pantry you will not be allowed to add any pet(s).  There is a maximum of SIX pets per household. All pets must be altered; you are required to show proof within 30 days. ONLY persons with authorized account holder’s ID will be given donations in place of Registered Recipient.

Registered Recipients will receive ONE (1) free food donation per month per animal. Donations are on a first come first serve basis, and we will accommodate allergies as best we are able. Any monetary donations received from a Registered Recipient will allow Recipient one (1) additional food donation per animal.  Further, I agree to not misappropriate, sell, abuse, or use illegally any donation received. Fraud charges and/or any other legal action(s) will be taken.
